19-5-2 Form and content of complaint.
The complaint shall be in writing on a form ob-
tained at the commission office. The original com-
plaint shall be signed and verified before a notary
public or other person duly authorized by law to
take acknowledgements. A complaint shall contain
the full name and address of the complainant and
the full name and address of the respondent. The
complaint shall also contain simple and concise
statements of the facts or information and belief
upon which the allegation of a violation is based.
It shall include, where known, the dates and
places of occurrences that are described and the
names of the participants and the section or sec-
tions of law which are alleged to have been vio-
