1.        Statement of Organization* (This form must be completed by the chairperson of each party committee which anticipates receiving contributions or making expenditures. The chairperson of each committee is required to file this statement not later than 10 days after the establishment of the committee or whenever there is a change in the information previously reported.

 

2.        Registration Statement* (The registration statement must be completed by the chairperson or treasurer of each political action committee.)

 

3.        Affidavit of Exemption From Filing Receipts and Expenditures Reports* (The treasurer of a political action committee which qualifies for the exemption may file this form. However you may not use this form if you received or expended or contracted to expend $500 or more or if you received a contribution in excess of $50 from any one contributor in 2021.)
